Massage Gift Certificate Instructions
Please follow these instructions to activate your gift certificate:
1. Click here to download the certificate.
2. Print the certificate.
3. On the blank line below my telephone number write one of the following:
30-minute massage
60-minute massage90-minute massage
4. Fill in the 'Presented To' and 'From' blanks.
5. Fill in the 'Please Schedule date'. You can schedule a date up to six months from date you purchase the gift certificate.
6. Fill in the blank after the W at the bottom right of the gift certificate as follows:
Last two digits of the year, followed by month, followed by day you purchased the gift certificate.
Example: January 7, 2008, would read: W-050107
Need a gift certificate at the last minute and my office is closed?
No problem! Leave me a voice mail message at 785.550.3852 with the following information:
Your name.
Your telephone number.
Length of massage you would like me to provide.
Name of person to whom you are presenting the certificate.
I will call you back when my office re-opens to obtain your credit card information.
PLEASE NOTE: THE CERTIFICATE WILL NOT BE HONORED IF PAYMENT IS NOT PROVIDED!
